I don't understand anymore

I really don't understand why our website is broken again. Some things work 100% of the time. Some things give me a 500 internal server error some times, but work on a shift-reload...other times they give me the wordpress 404 page, i.e. mod-rewrite is kicking in when it shouldn't, and sending things to WP, which works fine.

But why does it break
