you're supposed to go the registrar's office & just fill out a form that only asks for like your basic info (name, major, ID#, reason? (i always put personal haha) etc). at uml there's like a $10 (or was it $20?) withdrawal fee for paperwork or whatever~ but it's pretty easy lol doesn't even take ten minutes.
