autopsy of 23 patients with median age 69. Virus caused direct olfactory axonal injury ( myelin break, vacuolization, debris) + microvascular endothelial injury ( immune mediated)--> swelling, mitochondrial structural injury.
virus is detected in olfactory tissue in only 3 patients.
