It occurred to me while writing though, that maybe time is something that demons have control over in hell - so what we all think of as a standard universal "hell-time" isn't actually standard OR universal. They needed Dean to break and they wanted him to break fast, so maybe they manipulated time around him to make 4 months feel like 40 years, while the guy on two racks down from Dean, who didn't matter in the slightest, would have felt 4 months as 4 months.
Also, it would never have been Adam being rescued, because Adam isn't in hell - he's in the cage. :P It WOULD be nice if they'd tie up that loose end eventually though, I admit, but I knew it wasn't going to be in this episode.
True, I suppose it would have been nice if they had mentioned him. That being said, I can sort of understand why they didn't - if the writers aren't prepared to follow up on that storyline, they probably didn't want to remind casual viewers about the untied thread.

As for Adam, yes he is in the cage. And I didn't really think they'd rescue him, but they didn't even mention him! Oh well...
