I think it's best to use words that suit the character thinking/uttering them. And as a reader, I tend to find clinical words like vagina and penis jarring, I'd much rather either flowery euphemisms or vulgar terms.

About GRRM and smut - I think the problem people have is that they expect sex scenes to be sexy, while GRRM's sex scenes are not about titillating the reader - yes, not even the seemingly gratuitous lesbian scenes - they're about showing something about the characters involved.

I think because I am a modern person certain flower euphemisms cannot help but sound silly. I always say rampant manhood in my best Tim Curry voice regardless of the situation. Do you think Westeros people actually say rampant manhood the way we use erection or hard-on?
