 I've always had wonderful luck with USAA, my car insurance company.  They have always been super friendly and I have been satisfied with payments...unitl recently.  Not once, not twice, but three times did they sign me up (with my permission) for automatic withdrawl every month.  I am horrible about sending bills out on time so this option is

Nice of your insurance company to keep such good records about your payments and cancellations, but not so much about your automatic withdrawals. Unless they are idiots (which they may indeed be), *someone* in power should be able to reinstate your policy and (gasp) even apologize for your numerous inconveniences. Fight 'em, baby!
