well, i didn't ever need to worry about doctor's notes. I am on flexible schedule, and I don't require employees to be fixed to the hours as long as the job is done correctly and on time. Performance reviews on paper are in HR file, and I can store it on my hard drive, I didn't ever had PIP with anyone, so I have never found a reason, but they told us that we should keep minutes on 1:1 as well.
