Hook your PS3 up directly to your modem as a test. We found a setting with Decca's router that completely borked his Netflix streaming experience to similar to what you describe.
Has the line noise inside your house gotten any better? I remember that causing a fair bit of problems some months ago.
I'll have to try the direct connect test. It will take some shuffling to do so.
The line noise is gone. That was taken care of some time ago now. Turns out to have been caused by a lingering DSL filter that was no longer being used, so we removed it and it went away.
I can now maintain a consistent > 3.7Mbps when testing to "local" (ie: Seattle based) sources.
Curiously, what settings did you find that helped on Decca's setup?

Depending on what the client determines your bandwidth to be during its start up, it requests an appropriate quality stream. Sounds like you got one of the lower end ones. http://blog.netflix.com/2008/11/encoding-for-streaming.html
