The number one thing I can think of is businesses attempting to shape their entire strategy around an untested or unproven piece of tech or new methods. I am always shocked at what some companies are willing to abandon to jump onto some new idea that they don't fully understand yet because of buzzwords and empty promises. If the impact of the tech isn't neutralized by not being first to market with it, why rush? Let others be the guinea pig and learn from their mistakes.

The other thing which is related to this is understanding the problem that you're trying to solve.

1: We have an email problem with support.
2: What's the problem?
1: Well, our customers aren't happy with our email response time.
2: Here, let's sink thousands into our new email system!
1: Well, responses are still slow.
2: Oh, turns out our training was horrible and everyone was clueless as to how to do our jobs.
1: Oh well, we still have a shiny email system. *tiny flag*

I work for the County government and they make a good effort just to keep up with the times, but they are still slow and rely on the younger generation (aka mine) to sweep in and point them in the right direction and answer their questions. IE Twitter and facebook.

Currently in my division, I'm revamping about a few dozen excel files into a few with multiple tabs. This is cutting edge for my people. :sigh:

So you could go with make sure that all documents and much used files are actually streamlined to effectively use the functions of the program. Microsoft Office actually does more than you think it would.
