One time when I was on a roadtrip, my girlfriend asked me for some aspirin for a headache. I gave her my bottle of ibuprophen and warned her that they were 500mg each. Later she asked me much was in those and when I told her, she was like, "Oops. I took two." Way to go. No more medicine for you today.

And the worst part is probably no one at the hospital really believed that it wasn't intentional. Doctors are paid a certain stipend specifically to never believe anything any patient ever tells them. That's what they all go to Dr. House to learn.
