They could start by doing it like Australia:

If your fam. budget is above a certain amount you still pay... but under you don't. Reduce medical insurance companies rights to all the decisions.
And with the already free clinics and hospitals. Give funding there and reduce silly stuff from overpaid insurance paying hospitals!!

Don't hit the whole country in one go, start with communities in each state at a time. Ease them into it.
