I'd see this one around season 4-5 but it's up to you, really.
Sam and Dean aren't exactly seeing eye to eye, since Sam's doing his own thing and Dean doesn't exactly approve of his methods. Dean ends up acting angry most of the time, which leads Sam to believe that his brother doesn't care about him like he used to. Of course, if he were to get sick or injured, he would try to hide it from his brother, just so he doesn't end up pushing Dean away more than he already is.
RPF anyone? I never had the guts to try but I've had this prompt running around my head for a while. Anyone care to help? :)
Jensen/Jared: Jensen’s used to watch Jared act as Sam getting hurt and he’s used to being big bro Dean. That’s why he’s a little confused when he realizes that Jared’s not acting this time. He’s hurt for real (on set or something).
Sam is turned away when he tries to donate blood during college -- there's something Not Quite Right that they just can't figure out (it's the demon blood, although Sam obviously doesn't know it yet), so he waves it off and leaves the donation center, spending the rest of the day full of anxieties and confusions having to do with his feeling inherently impure. Maybe someone (Jess, Dean, a friend, a stranger?) is able to cheer him up a little and distract him from his insecurities.
