So Cas is in the bunker with the boys (no Gadreel) and Dean is kinda pissed about the angels falling, etc. So Sam spends more time with Cas as a result, since Dean's ignoring him.
Sam decides to teach Cas to drive.
And as a result, the Impala gets ... a SCRATCH. *gasp*
Cue furious Dean, tearful anxious Cas, calm mediating Sam - and ultimately, Dean and Cas working out their issues with Sam's help. :)
Sam hasn't been sleeping well. More importantly -- according to Dean, anyway -- Dean hasn't been sleeping well, on account of Sam's monster snoring. Finally, they figure maybe something's up, and Sam goes to a doctor, who diagnoses him with sleep apnea.

Early season 6, post "You Can't Handle The Truth." Both brothers are injured somehow. Dean's injuries are obvious and Soulless!Sam tends to them, much to Dean's chagrin. Soulless!Sam does not acknowledge his own - maybe he's unenthusiastic about accepting treatment from the guy who pulped his face so recently, maybe he just doesn't like to acknowledge weakness, maybe he legit doesn't care. Either way, they turn out to be reasonably problematic not long after and require care.

In his half-conscious state, he still clutched at his bleeding shoulder. There was a lot of blood, but since it was mostly his own, he made no comment about ruining the upholstery

Soul or not though, Sam’s work was faultless. He disinfected the wound with one swift motion. He then produced sterilized tweezers and dug the bullet out of Dean’s flesh. Sam was pitiless to his big brother’s groans. Still, his confident movements had the job done in the shortest amount of time they’d ever taken for a bullet wound. For a second, Dean wondered how many bullets Sam had dug out of people - or even of himself - in the past months. His brother did it like it was no big deal

Dean has been having a bit of a dry spell with the ladies. Hunting has sapped his....joie de vivre, so to speak. He decides to pop some Viagra before hitting a bar. Unfortunately, he strikes out. Not only does he have the boner that won't quit, it's that boner from the warnings - "if you have an erection lasting 4 hours or more, go to the doctor!". He's horrified to confess his problem to Sam, but he doesn't want to permanently injure his dick either. Finally Sam, aware that something is wrong but not knowing what, weasels it out of Dean. **Bonus points for first time wincest after Dean goes to the ER/doctor!
I'm just trying to picture Dean being totally awkward because he's hard like a horse, but still it's Sam he needs to ask help to!!! How traumatizing !!!!
Dean's been working really hard with his hands (digging graves, waving weapons at certain monsters, etc.) so they end up being really painful. Unfortunately, they're so painful he can hardly lift his toothbrush. He's going to have to try to work his own puppy eyes and very shamefully (and against his will) ask for his brother's help to get around simple tasks that require a pair of hands.
