We've seen Snape's, at the end of GoF.

We saw Pettigrew's, too. In the graveyard scene, Voldemort uses it to summon the other DEs.

As far as how it goes on, I think it's a spell, but it's not just *snap* and it's done, it's more complicated than that. So, a mixture of 'a slow, Muggle-like tattooing process' and 'a spell.'

I clicked Other for 2 and 3.

I think it's a combination of a brand/tattoo/something else. And I think it involves probably a spell/tattoo process/potion.

I've always thought of the Mark as a magical brand, but not necessarily quick. The Dark Lord would never let it be painless-- why deprive himself of so much fun?

I picked other for question 2. I think it's a spell, and therefore cannot be directly correlated with Muggle body mods like tattooing and branding.

Also, for question 5 I chose the inner circle from the graveyard, but it must be noted that they have not all been identified to us.
