I've tried using this layout--I chose no sidebar and horizontal advertising placement, and it ended up looking like this:

I think I chose the layout without a sidebar, apparently you still have to choose the one with a sidebar? ♥ Mine's all fixed now, I hope yours is too!

Are you using the sidebar or topbar version? Because if you use the topbar version, you should still chose the 'Flexible Squares' layout with a sidebar (left or right shouldn't matter) in order for the topbar to show up..

If that doesn't help let me know and I'll look into it some more to see if I can figure it out myself..
