When I looked at Recent Entries, I noticed that one of my userpics had been replaced by a picture I did not recognize.

I have had that happen a few times when LJ makes DB changes, in every case shift-reload has fixed the problem.

In addition to what volta said (or perhaps this is the same issue), there’s a similar problem which is more persistent. It only seems to affect particular people, and it’s predictable for them; the same userpic of theirs will get replaced with the same random userpic of somebody else’s, but it persists for hours or days. Dunno why, but I know it’s been around for a good long while.

my roommate had that happen and thought someone had hacked his pics.
it sounds like it is a LJ problem tho.
