Tony died.

Exhibit A: He eats an orange earlier in the episode. Characters in the series were often seen eating oranges shortly before danger.

Exhibit B: Tony and Bobby once had a conversation with one of his crew about what it would be like to get whacked. They agreed that things probably "just go black" and that you don't actually hear it happen - like the episode's end.
