Also depends on how it's made. If it requires the use of some whacky machine -and- it's under patent, there's nothing to stop them charging it at the full cost of production plus profit, especially if they know some hospitals will pay it. I think we forget, having most things subsidised, how expensive a lot of pharmaceuticals actually are. Plus, as you say, R&D ain't cheap.
If it's the thing I think it is it's made by extracting a tiny active bit out of blood people have donated by processing masses of blood. Anything like that is bound to be expensive.
