Do you intend to go for a Wi-Fi enabled Modem/Router or would a plug-n-slug one do for you?
If you're in Bangalore, I suggest trying to tap out a post on bangalore. That would probably be your best bet as of now.
Oh, and once you're done with the whole purchase/installation, do tell us what you ended up choosing and (if possible) also why. :)

1. Take the one provided by BSNL - Helps when you have to contact customer support. They're only well versed with modems they provide.
2. Installation guys should configure it. Again, refer my point above.
3. Doesn't matter much. I'd say go for Rental.
4. I don't really remember the difference, but go with the one that has four ethernet ports and built-in WiFi (must be Type IV). This might seem costly and unnecessary but it's worth it. If you already have WiFi router, go for the one with Ethernet and USB port (I think Type II).

If we don't have a Modem from BSNL they will simply refuse to provide support during network outage.
I opted to buy it than rent since i would end up paying (500 deposit + 250 installation charge + 50 monthly rental) vs (1200 to buy the modem from BSNL + free installation + no deposit + no rental)
Yes i know. if something happens to the modem then i would in trouble. But for most of them using BSNL told me that they have not faced any problem with the modem and have been using it since BSNL launched their service some years ago...
