I'd have to say the original texas chainsaw massacre. I saw that movie when I was 5 years old (evil father I had) and it scared the living shit out of me that I had nightmares for a few years. Just watched it recently and it still scared me in some parts. *shivers*
Didn't catch that one till I was almost grown and by then, I'd already seen the New Generation one with Matthew McConaughey and his remote control leg. AHAHAHAHAHAHHAAAAA
The Ring was pretty scary, but most people will tell you that it's an "experience" movie. In other words, you have to be in the right setting. The theater was nice and so was sitting at home alone in the dark.
But I don't get scared at movies very easily.
Amytiville Horror always got to me because it was supposed to be a true story. The first Poltergeist gives me the heeby jeebies. Alien is a good one, but it's more of an aged sci-fi movie at this point.
The "Based on a True Story" ones always get me, that's why I watch Blow once a week. The Amityville Horror, it's such a classic. And made before Margot Kidder went coo-koo!

I was TRYING to say that the following scared the bejesus out of me the first time I saw them:
Alien (I think maybe the scariest film) Halloween (just the music alone sets the hairs up) The Blob (granted I was 6 at the time and it gave me nightmares for years) The Fog (John Carpenter version, had me in the cold sweats when I was about 15)
I haven't seen The Fog, but I added it to the list. For some reason, this made me think of The Hitcher with Thomas C. Howell. You know, the one where the hitchhiker ties his girlfriend between 2 semi-trucks then BYE BYE girlfriend! It was great.
as far as visual imagery, i'd have to say the shining. the image of those girls in the hallway stuck with me for so long. even as an adult, reaching the end of a hallway that then turns to another hallway, i look over half-expecting to see them standing there. :/
psychologically, signs fucked with me pretty badly. i think he does a masterful job of making you scare yourself without the use of scary imagery. i still expect one day to hear those thud-thud-thud footsteps on my roof one night.
random confession of the day: i was really am kinda afraid of the dark.
