I actually know the answer to your question. They subscribe to a service that filters the mail for them. The service uses a combination of black lists and baysian algorithms (learning algorithsm) to decide what should be blocked and what shouldn't be. The particular service spring arbor uses is one of the better ones on the market, but there is no perfect solution.
