This is as big as a wallpaper, but it is a LOT easier to work on a big canvas for this, otherwise it gets too bitsy if it's too small. You can make it into an icon/s after you are finished.
Start with this for your bg:
Paste this into your image twice. Desaturate the first one, (Crtl+Shift+U) and leave at normal, set the second to color burn at 100%.
Take this and put it somewhere to the right.
Set it to darken.
Erase any parts you don't want, then if you want to, duplicate it once like I did.
Flatten this image and paste it into a new 1024X768 image with a white background.
Take any picture (big though, preferably) and desaturate if it isn't already.
If it has a white background, leave it, but if not, erase it all and either have it erased or white.
Put it somewhere on your image, and set this to multiply. You can erase anything you don't want etc.
Take another image, again, with a white bg if you can, or else erase it. Leave this one in color.
This image is kinda huge and I resized it a lot.
Set this one to darken and place beside the B&W image.
Again, erase anything you aren't happy with. I got rid of some of her head so it didn't look so cut off.
And that's the finish of that, you can resize it now, select certain parts etc. to make a 100X100 icon from.
