As for the car crash - did you cause damage to the other car that you'd have to pay for? I really hope not, because it sounds like you have enough on your plate already...
As for the appointment...is there anyway that you can go to a doctor in your area? You don't even have to tell them what the surgery is for...they just need to sign off on the paperwork and do the blood tests. Or you can call the Mazzoni and try to explain your situation, and see if they can accomodate you somehow to get in for next week.
And one more thought...perhaps it's possible to find a friend whose car you can borrow? Or even see if you can get someone to rent a car for you (because you have to be 21 to rent a car).

the front end of my car ended up ripping off and ended up lodged in the back end of her car, but insurance will pay for it, only not really because my rates are going to sky rocket which sucks
