I used this book here which comes with everything you need.

You can also download the forms from here.

After you file, you will have to wait three months before you can go in front of a judge & finalize it, FYI. He does not have to be there.

Good luck to you! It is rather simple when it's uncomplicated however there are certain things that have to be said in a certain order when you're in front of the judge. I was lucky my judge asked me the questions instead of making me read it.

I did my divorce in Maryland, but as far as I know, most states require physical separation for atleast a year before you can file for a final divorce. Total cost for me to file pro se was around $200. Mine was harder than some though because it took me awhile to find my ex (we'd been apart 7 years)
