Hm, I could use ticky boxes instead of ticky circles here. I had massages where I kept all my clothes beneath the waist on, massages where I only kept my underpants on and massages where I removed all my clothing. Depending on the kind of massage/situation and the preferences of the person giving the massage and with both man and woman as massage therapist/physiotherapist. ETA: since I can only choose one I take the one which tells you how much clothing removal I feel okay with personally for a massage. since I guess that your question is directed that way.
I think my habits are pretty similar to the way I behave in a sauna. Usually I would use my towel there to sit/lie on it. But if I get in there with people who seem to do everything to keep themselves wrapped inside the towel as much as possible, I might wrap the towel around myself to avoid making them uncomfortable. Same with the beach. On a "normal" beach I wouldn't sunbathe without wearing bikini bottom AND top, but I have been to nudist beaches in the past as well (as in the one without any clothing at all) I personally don't have a problem with nudity in certain situations, but I know lots of other people are a lot more "uptight" about it. So I guess, I adapt my behavior to my surroundings. Maybe it's just a coincidence, but in my experience people who grew up in East Germany are more relaxed in that area than people growing up in West Germany. And a lot more relaxed about it than some people in the US. I just say "nipple-gate"...
I usually just get neck and shoulder massages but the one time I had a proper massage the lady told me to leave my underpants on. I wore brand new underpants though because I didn't want her to judge me!
