When I had my 1912 dress (essentially a costume) made it ran me about the same price range for embroidery and everything, and that was after choosing the lowest priced option when it came to fabric. x.x

Generally, in a costume shop (i.e. for a theatre), they budget between 30-40 hours per costume that they're planning to build. My conservative estimate based on the picture is a minimum of 25 hours to have it done well, discounting the embroidery (although they might be able to find something similar that they could do as an appliqué, instead of having to do full-scale embroidery).

So if you want to pay a fair wage, figure $250-300 for labor/background tools required (i.e. providing the sewing machine/sheers/etc), and then the rest will depend on fabrics.

Granted, it's been a few years since I've worked in a shop, but that's my guestimate for you. Grain of salt provided free of charge ^_~

OK, re thyroid? The weight you lost is due to YOU. Thyroid weight falls off like, in a month or two, SERIOUSLY. Yours came off properly, slowly, due to your diligence. If your thyroid were hyper, you'd be WHOA skinny now, and WHOA unhealthy (if it were at the point where it made you lose weight). But don't think that you didn't do this - YOU DID. And it is awesome!

Look at what's going on in your life (lol this isn't "look at your life, look at your choices!" though it seems it started that way!). You've got a lot, you're looking at making major changes. Any of this could induce panic attacks!
