(hey, gang of flist - if you are reading this comment thread, please answer it before reading my spoilage below)

I see first to last:

The title, the hat/eyes as you said
then I see the death card, I barely notice the other cards -
THEN I see the gun.

As a graphic designer - et al -
I am interested in how this was designed - the artists for the pulps
were brilliant.

I think the intention is to see the gun last, so THE SHADOW kills the person as they are distracted by the other things first.
