The size is normally a "Free size/One size fits all" In Japan I know that it can fit from a size small to a medium. It might fight large too, but I am not too sure.

I normally use SAL since it is not too expensive and takes about a week to get there. I could use EMS but it tends to be more expensive.
Shipping with EMS could range from $12USD to $17USD if I use that shipping for the one item. Let me know.
If I were to do SAL it would be from $6USD to $10 USD
