Answer to #1: If they're really good, they belong in every category mentioned. And then some.
Answer to #2: Friendships that are mostly one-way are hard to maintain, they're just frustrating. Also unstable ones, ridden with jealousy, superiority -- wait, why am I even mentioning these as "friendships"? Well, maybe about 1% of the time they may be worth salvaging. If they're a great source of stress to me, usually I don't feel it's worth maintaining.
Answer to #3: Most people that come to mind weren't really good friends in the first place... and those that are/were, it was really hard to "let go", "drift apart", etc. but it most likely for the better. I could count these people on one hand.
