It was in a spot so obvious I'm shocked that I didn't look there earlier. I store a lot of random things in shoeboxes on some shelving I have in my closet. I thought I had gone through all of the shoeboxes, but I apparently had not. While looking for some old photos, I found a shoebox full of old software CDs and computer manuals. It was in there, safely in its own little jewel case. It looks like what I thought happened was exactly what did happen - in my move back from Boston, it got separated from the rest of the DVDs. I must put it in a separate jewel case to keep it safe, but forgot to transfer it back to its home.
