could it be your daily routine?
since humanity is very much a creature of habit, people tend to not go outside their comfort zones in their daily lives. They like status quo, to not shake things up. they have a set way of doing things and some tend to not deviate from that set schedule.
You say you can change those things about you.....
Weight loss is another routine you can develop. you change things in your daily life- eat different foods, start a routine of exercise...
Career? can't help there... but a routine can be developed to go back to school to learn a new skill... and skills are nothing more than routines and habits that people develop to do their job.
Friends? you have those. just call some people up and let's go out!
