When you're in profile mode, if you go to the friends tab, for lack of a better word, it will give you a choice of "add/remove". Click on that. It will bring up a page that shows the sites for everything you've joined and who you're friends with. At that point you can just click the name and it will take you there. Then add it to your bookmarks.
There is probably an easier way, but that's the only way I know to do it.

For starters, at the top of your page in LJ, there should be several links: "recent entries", "friends", "archive", and "user info". If you click on "friends" all the posts from the communities you joined, plus the individual posts from users you've added as "friends" will show up there.
