Of course, this wasn't 100% true and the real reason he picked up on it was because he was paying such close attention.
Bored and irritated with sticking his head inside the room to see nothing change, Karkat had finally dragged a chair in here and settled on it in the corner to wait instead. And finally seeing the moron move was a relief. Relief so he didn't have to keep sitting here like a dumbass.]

But to his credit he waited. He stood there silently, scowling and with arms crossed, at John's bedside as the human slowly began to stir. There was no sympathy or concern. Just irritation and an impatient tap tap tap of his finger against his arm.

[Which might not have been true. Karkat had no idea what had happened to him. He had been in pretty bad shape so maybe he had been near death. He didn't know how much a beating human could take. By troll standards those injuries were bad but probably not fatal. With humans? Who knew.]
