Then, once I am in shape, I feel so good and healthy that I worry about losing that good fitness level.

Have a work out buddy! Some one who can say "I'll see you there at X o'clock!" so you can't back out.

Running a certain distance. Or being a time. Or being able to lift a certain amount of weight.
Having those sorts of goals is what keeps me going.
That and the fact that i don't have cable so the gym is the only place I get to watch it.
