I enjoyed this very much - very creative, and I could visualise the Doctor as I read.

But can I recommend the community dw_britglish? There were a few instances where I was jerked out of the story by things which didn't fit the setting. For instance, in the UK healthcare is free at the point of use, so there would be no bill (unless Donna is in a private hospital, and I didn't get the impression at all that she was). Second, you don't tend to find volunteers giving directions and so on in British hospitals. There's an information desk, all right, but it's a permanent fixture rather than a table in a corner (as I've seen in North American hospitals) and operated by a hospital staff member
