Family History: Lillian Michaels (mother), hair stylist. Adam Ingram (father, not present). Dr. Conrad Michaels (grandfather, legal guardian), currently the Dean of Life Sciences at John Hopkins University.

Lillian became pregnant with Addison as a teenager, and she and Adam were not able to raise a child. It was decided that the best place for him was with his maternal grandfather, Dr. Michaels, who was granted custody. His mother had regular contact with him as he grew up. His father was absent from his childhood.

Medical History: Addison was born with oculocutaneous albinism (OCA1a). He has experienced no noteworthy injuries during his life, besides a failed suicide attempt (strangulation via hanging) two weeks prior to his admittance. He is underweight, which should be monitored.

Addison suffers from a series of complicated delusions, chief amongst which is that he is not human and that the end of the world is approaching. Addison claims that he has the ability to trigger his idea of the apocalypse. He believes that he was created artificially in a laboratory by his grandfather for this purpose. Addison thinks that many of the adults in his life are members of a secret government organization attempting to manipulate the apocalypse to cause all of humanity to join in a single consciousness. He refers to himself as "Kaworu" or "Tabris", and says that he is an angel. His religion seems to have become a large part of his paranoia, and frequently uses religious terminology when describing his paranoid delusions.

Addison is not aggressive, and is considered to only be a danger to himself. His suicidal tendencies seem to have receded for now, but may be aggravated again by exposure to stress. His initial suicide attempt was apparently triggered by the threat of separation from Sean Inoue, a classmate and close friend. It led to him becoming convinced that if he didn't kill himself, he would have brought about the apocalypse.

On the subject of Sean Inoue, Addison maintains an unhealthy fixation on him.

Current Status: Addison has not made any noteworthy improvements so far, although he has not made any further attempts to take his own life. He is still convinced of all of his delusions. He also believes that his suicide was a success and he's somehow been brought back to life.

He was visited by Sean Inoue in a brief, monitored session. He reacted well to it, and did not bring up his delusions. He appeared depressed for some time afterwards. His fixation has not improved either.

Nurses report that he's pleasant and agreeable, and seems to be socializing with other patients.

Working Diagnosis: Psychotic Major Depression, Schizotypal Personality Disorder

Life History (not part of patient file): Addison was a socially stunted child who had a difficult time relating to his peers. He had no close friends, and teachers complained that he did not put out effort in school. Dr. Michaels expressed concerns that Addison was bored in his classes and was failing them because he either wasn’t being challenged or had a learning disability. He made the decision to homeschool Addison so he could better address his grandson's needs. Addison took an interest in music, which he excelled at. He learned to play two instruments (violin, piano) at a professional level and to speak German fluently.

Addison reached high school age, and was mainstreamed into a private Catholic school. He became acquainted with Sean Inoue, the son of one of the researchers that worked for his grandfather. They interacted through a shared interest in music, and were highly praised members of the school orchestra. Sean Inoue suffered from severe social anxieties, but surprisingly Sean and Addison became very close. At first their respective families were relieved that the two were finally forming relationships, but soon became concerned that their friendship was unhealthy. The boys reportedly spent the majority of their time together and there were rumors at their school about the nature of their friendship. Both denied that there was any homosexual activity.

In addition to their unusually close friendship, Addison, Sean and several other classmates started participating in a series of complex fantasies. All of the children involved were notably very emotionally disturbed or suffered from their own problems, which was perhaps why they were drawn to one another. Addison joined the group as it was beginning to dissolve, and was quickly wrapped up in their stories. However, Addison took the “game” even more seriously, and became convinced that he was not human, and had been artificially created in his grandfather’s laboratory. His religious background also influenced his paranoia.

After several months of this, the Michaels and Inoue families mutually decided to try and limit the amount of time Addison and Sean spent together. The teenagers resisted the change, and the added stress seemed to aggravate Addison’s paranoia. When his grandfather told him he intended to transfer him to a different school, Addison planned to commit suicide. He called Sean immediately before the suicide attempt, and what he said was enough to motivate Sean to dial 911. The ambulance arrived in time to save Addison’s life, and he was hospitalized and put on suicide watch.

Since the incident, Addison’s delusions only became more complex and his behavior more unusual. He began to insist that if he hadn't tried to kill himself, then he would have brought about the apocalypse. As a result, his grandfather sent him to Landel’s Institute for aid.
