I ran across this style a while back, but still have no idea where to get it. I know its a custom style for paid users based off component, but that's about it.

That one is to make the entries squared instead of curved like in typical component style. That's the main tutorial you need.
Then you will need to have a fixed text box entry. I don't recall where that tutorial is. The closest I could find it here

But mainly you need the first set of tutorials for what you want.
