One Suggestion

I also feel that all the points are valuable. I am following most of the points, you suggested us to follow.
Just one tip I would like to give that is, keep immersing yourself in English environment. Speak as much as you can. Whether it is about anything. Hesitation is the biggest problem anyone has, and to get rid off by this, you just need to speak more and more.
These are very good habits! They are instructive and feasible.
If I were to add anything, I would say "balance". For example, "doing one or more of the above everyday" could mean watching English movies (and doing nothing else) everyday. But that would be far less efficient than, say, combining the watching with some speaking exercises. So I would suggest keeping a balanced way of learning be included in the list of good habits, because it seems equally important to me.

So stop thinking and start speaking.
All the best.
