I am writing in reference to account # 555-555-5555.
I am writing to determine what this is a bill for, if it is legitimately mine, and if the insurance has been processed properly before blindly paying it. (I tried resolving this by phone but your offices were closed).
According to the bill, this is for an Emergency Room visit to University Hospital in Other Neighborhood on August 19, 2013. I did visit the ER at Uuniversity Hospitaln in Actual Neighborhood, not Other Neighborhood.
Additionally, this was a Worker's Compensation claim, as I received a minor injury from a laboratory animal (mouse) and as such, am required by the University [we both work for] to be treated for minor injuries from laboratory animal bites at the ER at University Hospital in Actual Neighborhood should the injury occur after the employee clinic is closed. As such, I was under the impression that like my previous Worker's Compensation claim, I was not be charged for services that my employer insists I engage.
Therefore, before I pay this bill, I want to ensure that a) the Worker's Compensation claim was properly processed, b) that the insurance has properly paid these charges and c) I am not unnecessarily paying $125 because the University Animal Research Department required me to go to the ER to receive Neosporin and Band-Aid on a mouse bite because it occurred after 4:00 pm.
If you could confirm that the bill has been properly processed and I am indeed still being charged this money, I would appreciate it.
