I've seen some horrendous things that doctors have said to patients. I always think it looks really bad when it is quoted back in black and white. I know they have huge egos and it's hard sometimes to deal with the ordinary people but I can often imagine how these conversations go, with the doctors trying to communicate in the vernacular.

That woman has shit for brains. A big part of her problem is the modern urge to medicalize human behaviour: thus obesity becomes a disease of which the patient is a hapless victim; she need do nothing about it herself, just wait passively while the experts find a pill to cure it.
Again it's denial of physics: if calories consumed exceed calories burned, the result is fat. We all control what goes in our mouths.
Still, it was clearly unprofessional to swear at an (ex-)patient, but the NZ Health Commissioner has intervened on the wrong issue. (Is this a polly or a medic, btw?) The commissioner should be offered counselling. A raised middle finger should suffice.
