I'm not 100% sure on the post office part.

What I did was this...

I went to the post office locator, put in "Pittsburgh", "Pennsylvania", and my zip code (15215).

The post office with that zip code is technically in Sharpsburg, so even though I live in a different municipality (O'Hara), I put down Sharpsburg as my post office.

And I'd say that putting down your middle name shouldn't hurt. Even if your original voter registration wasn't filled out with your middle name included, I don't think it will cause a problem.
