I'm reading "One flew over the Cuckoo's Nest" and I'm wondering

WHAT IS THE FOG MACHINE?

Is it Chief physically going blind, or is it his way of putting his fears into an obvious state? Is it steam from the shower (or a kind of hormonal haze after being possibly raped?), fog from tears or what?
