It's a nickname! For the most part.

When Tokitoh is formally introduced to the storyline at the end of volume 1 it's Kubota staring at a passed out Tokitoh in the alley and the text reads "I picked up a stray cat today." Which is Tokitoh of course. The parallel plays it out a little bit more in a few ways. Kubota compares Tokitoh when he first meets him as a stray cat, because he shies away from adults and only seems to trust children. (He also claws Kubota's uncle!) He's also called a cat by Kou, the doctor who treats him when Kubota first finds him, because of his reluctance to go into Kou's shop (like an animal who doesn't want to go to the vet).

WELL as for embarrassing he probably had to be a little bit careful about that. There's a whole lot of wiggle-room for him to piss her off and have her keep him around due to her whole Trainer's Lust thing, but if he hurts her pride she might have had a worse reaction to that and just dismissed him. BUT WITH THAT IN MIND, I'm going to say something like in the M*A*S*H movie, except instead of everyone on the base laughing at her, they were just like "HOLY SHIT, COLONELLO. RUN. 8|"
