My fluffy, white dog, Angus, has been diagnosed with severe hip dysplasia. For a dog that is just over a year old, his case is bad. The Xray showed that neither of his hips are in joint. I would go with the 5k surgery- I know i've spent at least as much on ferrets in the last year or so- talk to your vet; some allow payment plans, mine allows me to post date checks (as in, they have a pile about an inch high of checks of mine, paying 50-400 a month, depending on the month.) That will help take the sting out of it.
