Stuff has always been falling off the rockets as they go into orbit, even if its just pieces of ice. This was never a problem before the shuttle because all the important bits were ontop of the rocket rather than strapped onto them. There have been bits falling off them even during the shuttle programme, they just only took notice when something fell off and fatally damaged the heat shield on Columbia.

You're right though, replacement is overdue and there's no real solution in sight :|
