Step 1

When I started making this icon, I had no idea what I wanted it to look like. Like almost always I started using a colour balance layer. (Boring, I know, I know)
These are the adjustments: I can't believe how ugly the greens are ;Q;
Shadows: 0; 0; -20
Midtones: -10; -20; -55
Highlights: 0; 0; +30

And because the greens look so ugly, create a new channel mixer layer. It's an awesome tool and I think more people should use it.
Reds: +115; +5; -10; +5
Greens: +15; +70; +40; 0
Blues clues: +7; +50; +90; 0
Step 3

The background doesn't look that nice either, so let's go colourful! enter rainbow vomit.gif here.
Choose a brush you like, and draw over the wall behind the Doctor. Make sure the opacity is very low, about 30%. (I used a brush of
this set from deviantart)
That's what I drew: Set it to ~screen.
Step 4

Attention! Again a
lifesaver texture! (by midnight_road)
Drag it ontop all your current layers and set it to ~overlay.
Now he looks like a proper alien EW! Add a mask to the layer and draw on it in different shades of grey until you think it looks nice.
This is my mask:

The face of the Doctor is far to bright now.
Choose a nude-gray-ish colour and draw over his face. Set the layer to ~multiply.
Step 6

The next steps are colouring, because it all looks very pale.
Add colour balance a g a i n.
Shadows: 0; 0; +12
Midtones: +5; -15; +30
Highlights: -10; +10; -15
To get rid of the purple shades, use selective colour and only change the grey tones:
Greys: +10; 0; +20; -2

Now, this is something for those who enjoy the paint-y style.
Use a brush from the set I linked to above and do something like this:

Sharpening:
Select all layers, merge them into a new one: Ctrl+Alt+Shift+E.
Sharpen and erase the parts you don't like/that are too sharp and you're done. :)
