I found this on Diaryland years ago, and thought I'd start spreading it around.

1. Who do you hide SI from? the offline world
2. Who knows about it? anyone who reads my LJ
3. How long has it been since you last cut? i'd say a week/week and a half
4. Have you ever tried to commit suicide before? yes
5. Where do you usually cut? stomach, arm
6. When you cut, who's usually the first to find out? SI communities
7. What's your worst experience with a fresh cut? i don't have bad experiences
8. Do you have a fascination with scabs? oh my god, yes
9. Do you like scars, yes or no? i adore them
10. Do you name your razor? i used to, when i had only one. his name was Floyd
11. What other methods of SI do you use? i used to not eat and hit myself
12. do you dislike the term "self mutilation"? no
13. What various ways do you use to hide cuts? arm socks (i know they're dumb, but you gotta do what you gotta do.)
14. Once cuts heal, do you still hide the scars? nope i wear them like clothes
15. Ever been institutionalized/hospitalized for SI'ing? not for that.
16. Do you ever run into problems with hiding cuts (i.e: gym change rooms)? it's winter, therefore, no.
17. What's the best part about cutting to you? the adrenaline, the calming affect afterwards
18. Do you know of any songs that talk about SI? wow, i don't think i know of a song that isn't, so i'll name bands. afi, mcr, the used, korn, slipknot, nine inch nails, marilyn manson, kittie...god, no wonder we're depressed.
19. Have you ever been caught cutting/burning, etc? ummm...years ago
20. What instrument do you use to cut? pencil sharpener blades are my life.
21. What causes you to cut? bad friends, lonliness
22. What do you feel afterwards? relaxation
23. What is your closest Close Call? never really had one
24. How long have you been doing it? 2 1/2 years
25. Do you keep a razor in your bag? every so often
26. Ever needed stitches for a cut so deep? yes, but i didn't tell anyone so i've just got dark pink scars.
27. Do you have someone like a therapist you talk to regularly? yep. i'm getting mighty sick of it.
