Are there deer where you are? I've frequently had them pull a plant up, and then decide they didn't want to eat it and so drop it back on the ground. That's about the only critter I can think of that would do that.
#2 - some times the soil settles and the plant doesn't settle with it, thus exposing the roots; sometimes, as in the case of iris, the newer sections grow upwards and climb on out of the ground. For iris, it's a good thing; for others, not so much. Just put some soil up around the crown to keep the roots under the dirt so they dont' dry out.

The biggest wild animal we've seen around is squirrels... only seen a rabbit once. LOL. Don't know if a rabbit could have done it or squirrel.
Thanks for the info. I will definitely be putting more soil out for my flowers to cover up the roots.
