Make the following icon;

. I got it as close as I could! I tried and I tried to get the jackets to match but I couldn't! I hope that it is close enough!

1. Open a new 100 x 100 image

2. Open your image and resize it. Drag it into your 100 x 100 image.
I got:


3. Duplicate your base and set to screen. Duplicate the screen layer twice.


4. Duplicate the screen layer (top layer) and set to soft light.
I got:


5. Make a new layer and fill with #F4DEA8. Set to multiply at 80% opacity. Erase where this layer covers her face.
I got:


(It looks really stupid right now, but I promise it'll get better!)

6. Now go to Layer > New Adjustment Layer > Curves
Set to:

RGB
Point One
Input: 50
Output: 87

Point Two
Input: 207
Output: 221


7. Now go to Layer > New Adjustment Layer > Hue/Saturation. Set the Saturation to +29.
I got:


8. Now I went to Layer > New Adjustment Layer > Selective Colour.
REDS
C: -21
M: +10
Y: +92
B: +15

YELLOWS
C: -100
M: +16
Y: +87
B: +8

GREENS
C: +100
M: -100
Y: -100
B: +68

CYANS
C: +100
M: -100
Y: -100
B: +38

NEUTRALS
C: 0
M: 0

BLACKS
C: 0

I got:


9. Now make a new layer and fill with #EFD69F. Set to Multiply at 50% opacity.
I got:


10. Now just to up the colour a bit, I added a new selective colour layer.
Set to:
REDS
C: -86
M:
Y:
B: -18

BLUES
C: 0
M: +100
Y: +100
B: +100

MAGENTAS
C: -100
M: +100
Y: -39
B: 0

BLACKS
C: 0
M: 0
Y: 0
B: +12
I got:


11. Now for the tricky bit. I opened a texture (made by me):

and erased where it covered her, although i left a strip down one side that was feathered slightly. If you need any help with this step let me know. It just takes a fairly steady hand and a little patience.
After all this, I got:


12. Now this is optional, but next I made a new layer and stamped the merged image onto it (Ctrl + Alt + Shift + E in ps7). Then I used the Dodge Tool to lighten up bits of the texture and image.
I got:


13. Now I added a little tiny text in a darker variation of the background colour.

14. Then I made a new layer, filled with #F3DFA7 and set to Multiply at 10% opacity.

And you are done!
