*cheers* See, that almost sounds useful. On the NCLEX, I got a lot of questions on "you're a clinic nurse, these people have called in, who do you call back first?" I just remember reading some of them and thinking, "Why the hell are they even calling the clinc? They should be heading for the ER!"

My personal favorite was the rectal foreign body. On abdominal exam, I noted vibration on deep palpation.

Nothing about adults ever again? Really?

So, as a family med person who treats the whole crew when they come in and the entire family has the same/similar complaints or if their ailments are inter-related, I've alway wondered... who diagnoses/treats/troubleshoots breastfeeding problems? Such as latch problems, or candida on the breast when the baby has thrush?

Yeah, I know--lactation consultant. But they can't prescribe galactogogues or candida treatments. Seriously. I've always wondered about this, because most of the OBs I've followed are bleeping morons when it comes to lactation, because they think once the baby's out, it's someone else's problem.

Of course, I'm also still mystified that the OB does the circumcision, when it's the pediatrician who's keeping up with whether or not the little peanut is peeing, and how the circ looks a week out... I've just got some old country doctor in me, I guess. :)

I do counsel for breastfeeding, a lot, and I've written Nystatin for a mom. My attending for continuity is the breast feeding guru of the hospital, and she's taught me a ton. As far as circs go, as I understand, peds does them in a lot of places, just not UK. If I were going to be a generalist, I would put some effort into learning how to do a good circ. But I'm thinking I won't do many in a specialty clinic.
